  • Have plateaued on semaglutide or another GLP-1 and need a different mechanism

  • Have no personal or family history of medullary thyroid carcinoma or MEN2 syndrome

  • Are not currently pregnant or breastfeeding

Tirzepatide may not be appropriate with a history of:

GLP-1 receptor activation

Is tirzepatide better than semaglutide?

For some patients, yes. For others, semaglutide is the better fit. Tirzepatide has shown greater average weight loss in head-to-head and comparative clinical data, particularly for patients with significant insulin resistance, higher BMIs, or goals above 15% weight loss. Patients with GI sensitivity or goals in the 10–15% range may do just as well on semaglutide with fewer side effects. Dr. Giffi evaluates both at your first appointment.

Can I switch from semaglutide to tirzepatide?

Yes, you may be able to, and it's a conversation Dr. Giffi has regularly with patients who have plateaued on semaglutide or whose goals have shifted. The transition is managed clinically, starting at an appropriate tirzepatide dose based on where you are in your program, not just where the standard titration schedule begins. If switching makes sense for your profile, Dr. Giffi will lay out exactly what that looks like.

How long does it take for tirzepatide to suppress appetite?

Most patients notice a meaningful reduction in hunger and food preoccupation within the first one to two weeks at starting dose.
